Emergency Information

Otis College of Art and Design is committed to the safety and well-being of our students, faculty, and staff.

The College assists in keeping the campus safe, but a safe campus can only be achieved through the efforts and cooperation of all students, faculty, and staff. This will assist you in working with the Otis community to ensure your safety and that of others.

The Student Handbook ("The HOOT") includes all emergency and safety information pertaining to Fire Safety, Lost and Found, the student Alcohol Policy, Effects of Drugs and Alcohol, and the Otis Code of Conduct. How to Report an Incident

Once we have responded to your immediate needs, we ask you to complete an student incident report form or a faculty incident report form if you are a victim or a witness to assault, theft, harassment, property damage, injury, car damage, vandalism, or other injury, accident, or crime not stated but similar in nature.

The Incident Report Form is available online by clicking the following link: Student Incident Report Form. Fill out the form as completely as possible, including any action taken as a result of the accident or incident. The Incident Report form should be filled out within 24 hours of the occurrence. Copies will automatically be sent to Facilities Management, Human Resources and Student Affairs when applicable.

If the incident or accident is life threatening, please call 911, and then contact Student Affairs at 310-665-6960.

  • All incidents will be investigated by the designated department head as appropriate (i.e. Student Affairs, Human Resources, or Facilities Management) and resolved within 30 days of occurrence.
  • A follow-up with the victim will be completed to document anything that needs to be corrected.
  • Any steps taken to assist or resolve the incident.
  • The on-line follow-up form will automatically be sent to designated departments when the form is submitted.
